- Gift Aid column — a tick box for each sponsor to declare Gift Aid
- Full address field — required for Gift Aid claims
- Date and amount columns — standard
- Gift Aid wording — the legal declaration at the bottom
- Paid column — to track who has paid

Gift Aid Declaration: I am a UK taxpayer and understand that if I pay less Income Tax and/or Capital Gains Tax in the current tax year than the amount of Gift Aid claimed on all my donations, it is my responsibility to pay any difference. By ticking the Gift Aid box, I confirm I want [Charity Name] to reclaim tax on this and any future donations until I notify them otherwise.
Going Digital
Paper sponsorship forms still work, but digital alternatives capture more donations:
- JustGiving / GoFundMe page — shareable link, card payments, automatic Gift Aid
- PledgeNow — QR code on your form links to a digital pledge page. Supporters scan, pledge, pay later.
- Combined approach — paper form for older supporters, QR code on the form for digital-first supporters
Tips
- Carry the form everywhere during the lead-up to your event
- Set a target and share progress — "I've raised £300 of my £500 target"
- Follow up on unpaid pledges — many sponsors forget to pay after the event
- Always include the Gift Aid column — it adds 25% for zero extra cost
